Objective - Ensure access to relief services to people with specific needs, in particular persons with disabilities, persons with severe or chronic medical condition, children, expectant mothers, mothers with young children, female heads of household and older persons in Compostella Valley Region To reach this objective, HI develops the Disability and Vulnerability Focal Point, mechanism. The strategy is to place in the heart of each affected community dedicated centers and to inform any vulnerable person and/or with disability victim from the crisis, she/he can receive welcome and services. This mechanism is characterized by the systematic implementation of the following activities: - Identification and registration of vulnerable people and their needs; - Assessment of the level of coverage of needs (basic and specific needs) using household and individual approach; - Mapping of existing services on the project area, their quality and their conditions of access, - Awareness/advocacy, advice, support to communities, stakeholders… on the inclusion of vulnerable people in their activities; - Information : what people can access and what they can do to get it; - Tracking of the beneficiaries internally and externally; - Database to monitor the identified needs and given services, and provide relevant information to others actors.
